We require the following information for monitoring and consent purposes.

- Adults: Please enter your place of employment (or "none") and your date / year of birth.
If eligible, you will receive free CSES membership through your school, college or employer.
Please note that participation in this activity is subject to the following terms:

- I/we agree to remain responsible for the participant(s) throughout the activity, including when travelling to and from the activity.

- Unless I/we contact CSES confidentially to raise a safeguarding concern (in which case the following will not apply), I/we consent to images and video captured at the event, which may include the participant(s) named above, being published by CSES without accompanying names. CSES will seek specific permission to publish images with names.
